Download, print, and fill these out before your visit. Bringing them filled in saves about 15 minutes at check-in.

Coming in for the first time.
Most first visits run 45β60 minutes - history, exam, and in-office testing the same day when indicated.
- Insurance card and a photo ID
- A list of all current medications - dose and frequency
- Prior imaging - CDs of prior echos, CTs, MRIs, or ultrasounds plus reports
- A short symptom timeline - when symptoms started, what makes them better or worse
- Your referring physician's contact info, if you were referred
- Completed new-patient forms from above

How to prepare for in-office tests.
Your provider will give specific instructions when your test is scheduled - those override anything here.
Echocardiogram
No special prep. Loose-fitting clothing. Eat and take medications normally. About 45 minutes.
Stress test (treadmill)
Comfortable clothes, athletic shoes. No caffeine 12 hours before. No food 2 hours prior. Ask about holding heart-rate or BP medications.
Nuclear stress test
No caffeine for 24 hours (including decaf and chocolate). No food 4 hours before. Bring a snack. Allow 3β4 hours total.
Vascular ultrasound
No prep for leg or carotid studies. Abdominal aortic: nothing to eat or drink for 8 hours. Two-piece clothing.
Holter / event monitor
Shower before - you can't get it wet afterward. Avoid heavy lotions on the chest. 30 minutes for placement.
Procedural visit
Detailed prep is provided 1 week before. General: arrange a ride home, no food after midnight, morning meds with a sip of water unless told otherwise.
Requesting a medication refill.
The fastest path is through your pharmacy - they handle the request directly with us.
- Contact your pharmacy and ask them to send a refill request to MVM.
- We respond within 1β2 business days. Don't wait until you're down to your last dose.

- Controlled substances may require an in-person visit before refilling. Plan ahead.

We accept Medicare, UnitedHealthcare, Blue Cross Blue Shield, Aetna, Cigna, Humana, and 20+ other plans across commercial and Medicare Advantage.

Requesting your medical records.
We process most requests within 5β10 business days.

- We’ll send you the authorization form to complete and sign.
- Return the signed form by fax, mail, or in person at either of our offices.
- We process within 5–10 business days.
Records being sent to another physician's office should be requested through that office's referral coordinator - we send directly to them.
Common questions.
Do I need a referral to schedule?
Most patients don't need a referral. Some Medicare Advantage and HMO plans require one - check with your insurance. If you need a referral, your primary care office can send it directly to us.
Do you accept Medicare?
Yes. We accept Medicare Part B and most Medicare Advantage plans. We're in-network with the major Florida MA plans.
How soon can I be seen?
Most new patients are seen within a week. Urgent or limb-threatening symptoms get same-week or same-day appointments - call and tell the team it's urgent.
Can I email Dr. Chainani about my symptoms or test results?
No - email isn't a secure channel for medical information. For anything clinical, call 305-279-2621. The front desk routes urgent questions directly to the team.
Do you offer telehealth visits?
Yes, for follow-up visits and medication management when appropriate. New patient consults and any visit involving an exam or in-office testing are in-person. Ask if telehealth is an option for your specific visit.
What if I can't make my appointment?
Call us at least 24 hours in advance. We'll find another time. No-show fees apply only after repeated missed visits without notice.
